  • All Assessments & Placement Test completely filled out (example FAS, SAT, FAIR testing scores).

  • A copy of the student's health and shot records (blue & yellow form).

  • A copy of student birth certificate

  • A copy of student current IEP (if student is ESE program).

  • Copy of student’s Psychological Evaluation.

  • A copy of parent/guardian photo ID and SS Card 

  • A copy of a Utility Bill in the parent name

  • Copy of student’s Medicaid and/or Insurance Card

  • Signed Parent Affidavit (notarized)

    • PLSA (Sworn statement)

    • McKay (Affidavit)

  • $550 Non-refundable Registration & Technology fee

  • Potential Student Form

  • Tuition Agreement

  • Diagnosis Confirmation (PLSA – Must be signed by Florida physician/psychologist)

  • FLDOE Identifying & Reporting Professional Misconduct Brochure

All information submitted will be kept by the Career Prep & Workforce Training Program of South Florida as part of the student's permanent file.
